Special Needs Certified Home Health Agencies (CHHAs) were introduced to reduce rehospitalizations. In 2018, Shining Star patients had a 3% rehospitalization rate compared to the State average of 6.43%

  1. Our small agency allows for a more individualized Plan of Care with greater patient attention

  2. All patients discharged from facility are seen for a Start of Care within 48 hours to ensure compliance with state and federal guidelines

  3. We serve patients of all ages, from pediatrics to geriatrics

  4. If eligible, the patient receives Home Health Aide services within 24-48 hours

  5. We provide additional care hours on a short-term basis for patients with existing Home Health Aide/Personal Care Aide services

  6. We assist with Medicaid application

  7. Fast-track to Nursing Home Transition and Diversion (24/7 coverage if patient is eligible)
