Skilled Nursing Services

Our Skilled RNs are the patient's primary care coordinator and will:

  1. Manage your plan of care, including scheduling visits from other Shining Star professionals

  2. Review your medical care needs on an ongoing basis and evaluate your recovery

  3. Keep your family, caregivers and doctors informed about your condition

  4. Make sure you understand all of your medications and take them correctly

  5. Teach you ways to manage your disease utilizing all resources available

  6. Assist with managing symptoms, including pain, by teaching the patient, family, or caregiver about the disease process

  7. Help you acquire and use medical equipment

  8. Check your vital signs, such as your weight, pulse, blood pressure, and blood glucose

  9. Review your overall condition

  10. Teach you and your caregivers about your disease or condition and how to spot changes that need further attention

  11. Teach you and your caregivers how to use special equipment (such as a blood pressure cuff)

Depending on your needs, your nurse may also:

  • Manage your medications

  • Assess how effective they are and monitor potential side effects

  • Work with your doctor and pharmacist to prevent dangerous drug interactions

  • Check to make sure your prescriptions are filled correctly

  • Administer injections

  • Monitor and care for your wounds, incision site, or sores

  • Provide targeted therapeutic interventions for slow-healing surgical wounds or chronic wounds such as pressure ulcers (bedsores)

  • Show you and your caregivers how to identify changes in your wound

  • Train you and your caregivers to care for your wound as it heals
